Combine points form World of Hyatt

In case anyone wants to combine Hyatt points (like from your SO's account). To do so, you'll need a form. If you Google it, the old Gold Passport form comes up, which they won't accept. It's the exact same form, but is blue and has "World of Hyatt" on it. You'll also need to send it to the World of Hyatt email instead of the Gold Passport one.
